Respiratory tract flora, skin flora, bowel flora

Ceftriaxone 100mg/kg/day IV div q12h (max 4000mg/day)

AND

Metronidazole 30mg/kg/day IV div q6h (max 500mg/dose)

AND

Vancomycin IV [see dosing recommendations in Bugs and Drugs Section]¹

Duration of Therapy & Clinical Pearls

Consult Infectious Diseases and Neurosurgery.

IV to PO Transition: Consider following a minimum 2-4 week IV therapy course, and only once demonstrating improved clinical exam, afebrile, normalized CRP.

Length of Therapy:

6 weeks minimum, depends on abscess drainage and clinical course.
