Mycobacterium tuberculosis
Drug-susceptible Pulmonary Tuberculosis:
Rifampin 20mg/kg/day PO div q24h (max 600mg) for 6 months
AND
Isoniazid 10mg/kg/day PO div q24h (max 300mg) for 6 months
AND
Pyrazinamide 30-40mg/kg/day PO div q24h (max 2000mg) for 2 months
AND
Ethambutol 20mg/kg/day PO div q24h (max 2500mg/dose) for 2 months¹
Latent Tuberculosis:
Isoniazid 10mg/kg/day PO div q24h (max 300mg) for 6 months

OR

Rifampin 20mg/kg/day PO div q24h (max 600mg) for 4 months
​
OR
​
Isoniazid 10mg/kg/day PO div q24h AND
Rifapentine once weekly for 3 months:

10 to 14 kg: 300 mg/dose once weekly

>14 to 25 kg: 450 mg/dose once weekly
-
>25 to 32 kg: 600 mg/dose once weekly
-
>32 to 50 kg: 750 mg/dose once weekly
-
>50 kg: 900 mg/dose once weekly²

For patients receiving isoniazid for latent TB treatment, serum transaminase levels do NOT need to be monitored before or during therapy except in patients with underlying liver or biliary disease or unless being taken concurrently with other hepatotoxic drugs.²
