Herpetic Gingivostomatitis

Herpes simplex virus

Valacyclovir (only if ≥ 3 months of age) 40mg/kg/day PO div BID (max 1000mg/dose) OR

 

Acyclovir 80mg/kg/day PO div QID (max dose 800mg)

 

 Severe disease/inability to tolerate PO:

Acyclovir 30mg/kg/day IV div q8h (dose based on adjusted body weight if obese)

Empiric Therapy & Alternative Regimens

Duration of Therapy & Clinical Pearls

Treatment recommended with anti-viral therapy within 72-96 hours of disease onset if presenting with significant pain or decreased PO intake as trials have shown treatment significantly reduces duration of symptoms.

 

Complications include: Herpetic whitlow or herpetic keratitis from autoinoculation, bacterial superinfection, eczema herpeticum in patients with atopic dermatitis
