Pelvic Inflammatory Disease

C. trachomatis, N. gonorrhea, and anaerobes

IV regimen for inpatient:

Cefoxitin 2000mg IV/dose q6h

AND

Doxycycline 100mg/dose PO/IV BID

 

Outpatient Regimen:

Ceftriaxone 250mg IM single dose

AND

Doxycycline 100mg PO BID for 14 days¹¹

Empiric Therapy & Alternative Regimens

Duration of Therapy & Clinical Pearls

Consider hospitalization and parenteral therapy in patients who have severe illness with high fevers, severe abdominal pain, nausea and vomiting preventing toleration of PO, PID complicated with pelvic abscess, are pregnant, or if there is concern for nonadherence to therapy.

 

For PID complicated by pelvic abscess, ADD Metronidazole 500mg PO/IV BID

 

Upon transitioning parenteral therapy to PO regimen, patient should complete a total 14-day course of doxycycline and a total 14 day course of metronidazole for those patients that require it as above.
